STATUS TERKINI KES DISAHKAN COVID-19 YANG TELAH PULIH

Kementerian Kesihatan Malaysia (KKM) ingin memaklumkan bahawa terdapat 140 kes yang telah pulih dan dibenarkan discaj pada hari ini. Jumlah kumulatif kes yang telah pulih sepenuhnya dari COVID-19 dan telah discaj daripada wad adalah sebanyak 7,873 kes (92.5 peratus daripada jumlah keseluruhan kes).

STATUS TERKINI KES COVID-19 DI MALAYSIA

Sehingga 17 Jun 2020 pukul 12:00 tengah hari, terdapat sepuluh (10) kes baharu yang telah dilaporkan. Ini menjadikan jumlah kes positif COVID-19 di Malaysia adalah sebanyak 8,515 kes. Oleh itu, jumlah kes aktif dengan kebolehjangkitan COVID-19 adalah 521 kes. Mereka telah diasingkan dan diberi rawatan.

Ingin dimaklumkan, tiada kes import yang mendapat jangkitan di luar negara. Daripada sepuluh (10) kes penularan di dalam negara, tiga (3) kes adalah bukan warganegara manakala tujuh (7) kes adalah di kalangan warganegara Malaysia.

Bagi kes jangkitan dalam negara (3 kes) di kalangan bukan warganegara adalah seperti berikut:

• Wilayah Persekutuan Kuala Lumpur: satu (1) kes kontak rapat kepada kes positif COVID-19 (kes ke-8455);
• Negeri Sembilan: satu (1) kes di Akademi Imigresen Malaysia, Port Dickson di mana kes ini merupakan Orang Kena Tahan (OKT) yang dipindahkan ke sana daripada Depot Tahanan Imigresen (DTI) Bukit Jalil;
• Sabah: satu (1) kes saringan pra pembedahan di Hospital Wanita dan Kanak-Kanak Sabah (HWKKS).

Perincian kes penularan COVID-19 dalam negara di kalangan warganegara Malaysia (7 kes) pula adalah seperti berikut:

• Selangor – lima (5) kes:
  • Empat (4) kes Kluster Warga Emas Kuala Selangor, iaitu kontak rapat kepada kes positif COVID-19 (kes ke-8490). Kluster ini akan diperincikan di bawah;
  • Satu (1) kes saringan pusat Tahfiz;
• Sarawak: satu (1) kes kontak rapat kepada kes positif COVID-19 (kes ke-8453). Beliau juga merupakan ahli keluarga kepada kes ke-8453;
• Sabah: satu (1) kes kontak rapat kepada kes positif COVID-19 (ke-8403 iaitu kes kematian ke-120). Kluster ini akan diperincikan di bawah.

Sehingga kini, sebanyak empat (4) kes positif COVID-19 sedang dirawat di Unit Rawatan Rapi (ICU). Tiada kes yang memerlukan bantuan pernafasan.

Adalah dimaklumkan bahawa tiada pertambahan kes kematian berkaitan COVID-19 dilaporkan pada hari ini. Justeru, jumlah kumulatif kes kematian COVID-19 di Malaysia adalah kekal sebanyak 121 kes (1.42 peratus daripada jumlah keseluruhan kes).

KLUSTER MDR (MENINGGAL DI RUMAH), SABAH

KKM ingin memaklumkan sebuah kluster telah dikenal pasti berkaitan dengan kes kematian ke-120 (iaitu kes ke-8403) di Sabah.

Kes pertama kluster ini (iaitu kes ke-8403) adalah seorang warga emas yang telah meninggal dunia di rumah pada 12 Jun 2020 akibat komplikasi sakit jantung dan telah dibawa ke Jabatan Forensik Hospital Queen Elizabeth. Saringan COVID-19 telah dijalankan dan beliau didapati positif. Sebelum ini beliau telah mendapat rawatan untuk penyakit jantung di Hospital Queen Elizabeth 2.

Aktiviti saringan kontak rapat telah dijalankan dan didapati dua (2) kakitangan wad adalah positif COVID-19. Sehingga 17 Jun 2020 jam 12 tengah hari, seramai 63 individu (termasuk 21 anggota kesihatan, dua (2) pesakit, 40 ahli keluarga dan rakan) telah dikenalpasti serta disaring.

Langkah pencegahan seperti proses nyah kuman dan nyah cemar telah dijalankan di semua tempat yang dikenalpasti terdedah termasuk di wad dan rumah arwah. Perkhidmatan Hospital Queen Elizabeth 2 tidak terjejas dan berterusan seperti biasa. Punca jangkitan kluster ini adalah masih di dalam siasatan.

STATUS TERKINI KLUSTER WARGA EMAS KUALA SELANGOR

Sehingga 17 Jun 2020 jam 12 tengah hari, seramai 64 orang telah disaring untuk COVID-19 di sebuah rumah jagaan warga emas di Kuala Selangor setelah satu kes positif COVID-19 dikenal pasti penghuni rumah jagaan ini . Saringan ini termasuk:

• 12 kakitangan;
• 24 penghuni;
• 10 ahli keluarga;
• 13 petugas kesihatan; dan
• Lima (5) orang awam.

Daripada saringan tersebut, didapati:

• Tujuh (7) kes positif COVID-19;
• 43 kes negatif COVID-19; dan
• 14 masih menunggu keputusan.

Langkah pencegahan seperti proses nyah kuman dan nyah cemar telah dijalankan di pusat jagaan tersebut. Kakitangan pusat jagaan telah diberi taklimat tentang amalan pencegahan seperti penjarakan sosial yang selamat dan pemakaian PPE semasa pengurusan penghuni.

Pusat ini masih beroperasi seperti biasa dan penghuni telah dikuarantin di pusat ini. KKM akan terus memantau keadaan kesihatan penghuni rumah jagaan warga emas tersebut. Mereka yang bergejala akan dimasukkan ke hospital untuk rawatan selanjutnya.

PERKEMBANGAN AWAL KEBERKESANAN UBAT DEXAMETHASONE DALAM MERAWAT PESAKIT COVID-19 DI HOSPITAL

KKM mengambil maklum perkembangan awal ujian klinikal (iaitu randomised clinical trial) ubat Dexamethasone dalam rawatan COVID-19 yang dikenali sebagai “Recovery Trial” di University of Oxford, United Kingdom.

Dexamethasone adalah sejenis ubat steroid yang digunakan untuk mengurangkan radang (inflammation) dalam pelbagai penyakit. Hasil dapatan awal Recovery Trial menunjukkan ubat ini dapat mengurangkan risiko kematian iaitu sehingga satu pertiga dalam kalangan pesakit COVID-19 yang mempunyai gejala pernafasan yang serius, terutamanya mereka yang memerlukan bantuan pernafasan (ventilatory support).

Namun begitu, ujian klinikal ini juga mendapati bahawa ubat ini tidak membantu dalam rawatan pesakit COVID-19 yang hanya mempunyai gejala ringan atau pesakit yang tidak memerlukan bantuan pernafasan.

Hasil penemuan saintifik ini penting dalam perkembangan rawatan pesakit COVID-19. KKM ingin mengucapkan syabas kepada kerajaan United Kingdom, University of Oxford dan semua anggota yang terlibat dalam uji kaji klinikal ini.

Walau bagaimanapun, KKM menyeru orang ramai supaya berwaspada agar tidak menyalahgunakan atau mengambil ubat dexamethasone tanpa preskripsi daripada doktor. Ini kerana ubat ini juga mempunyai kesan sampingan dan perlu diimbangi dengan keperluan penggunaan ubat ini bagi penyakit-penyakit tertentu.

NASIHAT KESIHATAN BERKAITAN COVID-19

Orang awam secara amnya juga sentiasa diingatkan untuk mengamalkan nasihat kesihatan yang kerap kali diingatkan oleh KKM iaitu:

• Elakkan 3S / 3C:iaitu elakkan kawasan yang sesak (Crowded place), elakkan kawasan yang sempit (Confined space) dan elak sembang atau bertutur dengan jarak yang dekat (Close conversation).

• Amalkan 3W: iaitu kerap mencuci tangan dengan air dan sabun (Wash), sangat digalakkan memakai alat pelitup separuh muka (penutup hidung dan mulut – face mask) di tempat awam atau jika bergejala (Wear), dan (Warn) iaitu amaran dari KKM yang lebih berkonsep peringatan seperti:
  • Elak bersalaman / bersentuhan;
  • Amalkan etika batuk dan bersin;
  • Dapatkan rawatan segera jika bergejala;
  • Duduk di rumah dan elakkan kunjung-mengunjung;
  • Lakukan aktiviti disinfeksi dan pembersihan di kawasan yang kerap disentuh.

Selain itu, sentiasa ingatlah kepada S.O.P, iaitu:

• S:Syarat-syarat seperti yang ditetapkan di bawah PKPB;
• O:Orang yang berisiko tinggi seperti kanak-kanak, bayi, warga emas dan orang kurang upaya (OKU) perlu dilindungi, individu tidak sihat dan bergejala dikehendaki segera menjalani pemeriksaan kesihatan;
• P:Penjarakan sosial (social distancing) diamalkan pada setiap masa iaitu sekurang-kurangnya 1 meter.

KKM akan terus memantau perkembangan kejadian jangkitan di dalam negara, dan juga luar negara melalui maklumat yang diperolehi daripada pihak Pertubuhan Kesihatan Sedunia (WHO). Orang ramai akan sentiasa dimaklumkan sebarang perkembangan terkini mengenainya. Begitu juga, KKM akan memastikan langkah pencegahan dan kawalan yang sewajarnya adalah dilaksanakan secara berterusan.

UPDATES ON THE CORONAVIRUS DISEASE 2019 (COVID-19)

SITUATION IN MALAYSIA

Current Status of Confirmed COVID-19 Cases Who Have Recovered

17 June 2020 – The Ministry of Health (MOH) would like to inform that 140 cases have fully recovered and discharged well today. Cumulatively, 7,873 confirmed COVID-19 cases have fully recovered (92.5% of total cumulative cases).

Current Situation of COVID-19 in Malaysia

17 June 2020, 12 pm – A total of ten (10) additional confirmed COVID-19 cases were reported to the National Crisis Preparedness and Response Centre (CPRC) MOH today. Cumulatively there are now 8,515 confirmed COVID-19 cases in Malaysia. Therefore, there are currently 521 active and infective COVID-19 cases. They have all been isolated and given treatment.

No imported cases from overseas are reported today. Of the ten (10) local transmission cases, three (3) cases are non-Malaysians and seven (7) cases are among Malaysians.

Details for the local transmission cases among non-Malaysians (3 cases) are as follows:

• Kuala Lumpur – 1 case: Close contact to a confirmed COVID-19 case (Case 8,455).
• Negeri Sembilan – 1 case: Detainee at the Malaysian Immigration Academy who was transferred from the Bukit Jalil Immigration Detention Centre.
• Sabah – 1 case: Pre-surgical screening at the Women and Children’s Hospital, Kota Kinabalu.

The details of the COVID-19 local transmission cases among Malaysians (7 cases) are as follows:

• Selangor – 5 cases:
  • 4 cases: Senior Care Facility Cluster in Kuala Selangor i.e. close contacts to a confirmed COVID-19 case (Case 8,490). The detail of this Cluster is described below.
  • 1 case: Screening at a religious school
• Sarawak – 1 case: Another close contact (family member) to a confirmed COVID-19 case (Case 8,453).
• Sabah – 1 case: Another close contact to a confirmed COVID-19 case (Case 8,403, death #120). The detail of this Cluster is described below.

Currently, four (4) confirmed COVID-19 cases are receiving treatment in intensive care units (ICUs), and no patient is on ventilation support.

Fortunately, no additional COVID-19 death was reported to the National CPRC MOH today. Cumulatively, there are now 121 COVID-19 deaths in Malaysia (1.42% of total cumulative cases).

COVID-19 Cluster in Sabah

MOH would like to inform that a new cluster has been identified in connection to Death #120 (Case 8,403) in Sabah.

The first confirmed COVID-19 case (Case 8,403) was an elderly woman who died at home on 12 June 2020 due to complications of heart disease and was brought to the Forensic Department of Queen Elizabeth Hospital. COVID-19 screening was done, and she was found to be positive. She had previously received treatment for heart disease at Queen Elizabeth 2 Hospital.

Screening of close contacts was done and two (2) healthcare workers (HCWs) in the ward were found to be positive for COVID-19. As of 17 June 2020, 12 pm, a total of 63 individuals (21 HCWs, 2 patients, 40 family members and friends) have been identified and screened.

Preventive measures such as cleaning and disinfection have been carried out in all the identified high-risk areas, including the ward and the deceased’s home. The clinical services at Queen Elizabeth 2 Hospital are not affected and continue as usual. The source of infection for this cluster is still under investigation.

Current Status of the Senior Care Facility Cluster in Kuala Selangor

As of 17 June 2020, 12 pm, a total of 64 individuals have been screened for COVID-19 at a senior care facility in Kuala Selangor after a confirmed COVID-19 case was identified among one of the residents. The individuals screened were:

• 12 staff
• 24 residents
• 10 family members
• 13 HCWs
• 5 members of the public

From the screening, the results were:

• Seven (7) confirmed COVID-19 cases
• 43 individuals tested negative
• 14 are still pending results

Preventive measures such as cleaning and disinfection and have been carried out at the senior care facility. The staff of this centre have been briefed on preventive practices such as safe social distancing and the use of personal protective equipment (PPE) when managing the residents.

The senior care facility is still operating as usual and residents have been quarantined at the centre. MOH will continue to monitor the health status of the residents. Those who develop symptoms will be hospitalised for further treatment.

Early Report on the Effectiveness of Dexamethasone in the Treatment of Hospitalised COVID-19 Patients

MOH is aware of the recent development of a randomised clinical trial on the use of Dexamethasone in the treatment of COVID-19, known as the “Recovery Trial” at the University of Oxford, United Kingdom.

Dexamethasone is a steroid drug used to reduce inflammation in many diseases. Initial findings of the Recovery Trial showed that this drug reduces the risk of death by up to one-third amongst COVID-19 patients with serious respiratory symptoms, especially those requiring ventilatory support.

However, this clinical trial also found that this drug was not effective in the treatment of COVID-19 patients with mild symptoms or in patients who do not require ventilatory support.

This important scientific finding is important in the evolving management of COVID-19 patients. MOH would like to thank the Government of the United Kingdom, the University of Oxford and all parties involved in this clinical trial.

However, MOH urges the public not to abuse or take dexamethasone without prescription from a doctor. This is because this drug also has side effects and must be judiciously used in treating certain diseases.

Health Advisory on COVID-19

The general public is also reminded to always follow the health advisories repeatedly mentioned by the MOH, that includes:

• Avoid the 3Cs: Crowded places; Confined spaces; and Close conversations

• Practice the 3Ws: Wash hands frequently with water and soap; Wearing facemasks are strongly encouraged in public areas or if symptomatic; Warn self and others for the following, in line with MOH’s advisories:
  • Avoid shaking hands or touching others
  • Practice good coughing and sneezing etiquette
  • Seek early treatment if symptomatic
  • Stay at home and avoid from visiting others
  • Regularly clean and disinfect commonly touched surfaces in common areas

In addition to the SOPs, continue to adhere to THiS:

• T: Terms set under the Recovery Movement Control Order (RMCO)
• Hi: High-risk groups such as children, infants, older adults and the disabled must be protected, and if unwell with symptoms, to seek early treatment
• S: Safe social distancing is practiced at all times, of at least 1 metre away from others

MOH will continue to monitor the development of the COVID-19 situation, both in the country as well as internationally, based on available information from the World Health Organization (WHO). The public will be continuously updated on the latest information. In addition, MOH will ensure that appropriate preventive and control measures are continuously implemented.
